Medical Aesthetic Nurse Sharen McBride discusses her experience with microneedling

What is microneedling?
Microneedling is a minimally-invasive treatment which causes controlled trauma to the skin by puncturing the epidermis—and depending on the needle length, also
penetrates the dermis—with fine needles contained in a sterile cartridge. This triggers the body’s own repair process and wound healing mechanism, increasing collagen production without causing over contraction of the skin.
The puncture channels close quickly, after allowing the infiltration of topical active ingredients of the products used in the treatment to penetrate into deeper layers of the skin.

Which skin conditions benefit the most from microneedling treatment and why?
Microneedling is used to benefit skin that is prematurely ageing to treat fine lines, and for mature skins showing skin laxity, enlarged pores and wrinkles. It will improve the appearance and texture of the skin. Microneedling is also used to treat acne scarring and skin that has been damaged by over exposure to UV rays. Other types of scarring can be treated with microneedling including stretch marks and burns scars. The body can be treated as well as the face, neck, décolleté and hands. Treatment protocols are also available for hair restoration.

Is microneedling suitable for all skin types?
There are some contra-indications to treatment. Although skin microneedling can be performed on all skin types, skins with active conditions such as acne or infections cannot be treated.
For Fitzpatrick phototypes above IV, preparation should be used to prevent the possibility of hyperpigmentation rebound, and if treating hyperpigmentation then homecare products are recommended to be used for the duration of the treatment programme.

There is a full list of contra-indications available to Wigmore Medical account holders within the XCellaris PRO TWIST Sync folder.

What other aesthetic treatments compliment microneedling?
Combination treatments are performed in many clinics—the most common being microneedling with chemical peels or platelet rich plasma (PRP). There are no protocols yet from the manufacturer for combining treatments.

Why did you choose the XCellaris PRO TWIST device for your practice?
The XCellaris PRO Twist has been developed by the manufacturers of Dermaroller, who have over 20 years’ experience in microneedling, and it is classified as a class IIa medical device.
It has the newest in motor powered technology, and gives me confidence that the treatment experience and outcome will be favorable for my patients.

The handpiece has an adjustable penetration depth of 0.25mm to 2.5mm, while active needle retrieval and the needle count in the cartridge ensure atraumatic penetration, minimising any pain felt by the patient. The device therefore delivers a comfortable treatment which means there is less need for topical anesthetic. The easy-to-hold handpiece is also comfortable for the practitioner, especially when performing several treatments in a day.

There are protocols for several treatments with the device, as well as consultation and medical questionnaires, consent forms, and pre and post patient advice forms, ensuring that everything is in place to perform the treatment. All these are available within the XCellaris PRO TWIST Sync folder—accessible to Wigmore Medical account holders.

How does working with Dermaroller products benefit your business?
The products have been specifically designed to work with the treatment, and the ingredients in each product are specific to treatment indications. The range is not too extensive, and there are products to use during the treatment as well as products for patient to purchase for home use. The active ingredients Matrixyl 3000 and Copper Tripeptide-1 are present in most of the skincare and treatment products, which enhances the outcomes. To be able to offer everything the patient needs for at-home use is a benefit.

What is your approach to combining microneedling treatments with skincare products?
I believe that there is little point for a patient to spend time and money on a treatment without also investing in the correct skincare products. For a sustained result the active ingredients of these products should complement the treatment, with the aim to repair, regenerate and replenish the skin—as highlighted by the XCellaris PRO Skincare.
I provide a consultation prior to treatment to determine the patients needs so they can be advised on the appropriate skincare products for them to use pre, during and post treatment.
